Checking the status of your claims, as well as filing and reporting it has been made so convenient and efficient through our online claims submission form.

You should consider filing a claim as soon as possible in case of the following scenarios:

• A person gets injured.
 
• You incurred damage/s to someone else’s property or vehicle.
 
• You experience damage/s to your property or vehicle.
Step 1: Report a claim via MICI Customer Service Center at 867-2888 or send an email to [email protected]. You can also touch base with your insurance agent or visit us at 3/F, Athenaeum Building, 160 L.P Leviste St., Salcedo Village, Makati City, Philippines.
 
Step 2: A Claims Representative will get in touch with you within 24 hours to get the incident details, as well as walk you through the claims process and requirements.
 
Step 3: Submit all requirements immediately. If there are any additional requirements, our claims representative will connect with you.
 
Step 4: You will receive status confirmation from our claims representative, such as the decision for your approval, once everything is agreed upon by all parties involved.
 
Step 5: Your LOA/claims settlement is ready for release once your claims report is approved.
You may check your claim status for the following types of claims – Motor Vehicle, Fire and Property, and Personal Accident.
Some policies, such as fire and property insurance, will take a longer time to claim than others. Generally, if you have any plans to file a claim, you should do it as soon as now.
Yes, only original Claim documents will be accepted. You may submit all necessary documents to our MICI Customer Service Center or through your agent. You may also visit us at 3/F, Athenaeum Building, 160 L.P Leviste St., Salcedo Village, Makati City, Philippines

Claim Document Checklist

A. General Requirements
1. Incident Report and /or Notarized Affidavit of Loss;
2. Statement of Claim – this include the amount being claim;
3. Proof of ownership of the affected property (i.e. BIR  2303 Cert. of Registration, Mayor’s &/or Business permit, lease contract agreement, or any other document bearing the name of the owner;
 
B. For Damaged Leasehold Improvements
1. Repair / Replacement quotation;
2. Proof of repair / replacement (e.g. sales invoice, delivery receipts &/or official receipt, Purchase Order);
3. Photographs showing the damages to the property;
 
C. For Damaged Stocks/Inventory Claim
1. A detailed inventory of the affected items indicating the extent of damage and its unit cost;
2. A detailed inventory of stocks at the time of the loss;
3. Proof of Costing/Pricing – Sales Invoice, Delivery Receipts, Official Receipts Pricing Catalogues;
D. For Damaged Electronics Equipment*, Furniture, Fixtures, Fittings, Office Contents and Equipment   
1. Repair / Replacement quotation;
2. Proof of repair / replacement  (e.g. sales invoice, delivery receipts &/or official receipt);
3. Copy of technical analysis report from technician with regards to the nature / possible cause of damage, extent of damage and recommendation if repairable or beyond repair for electronic equipment *
 
NOTE: OTHER DOCUMENTS MAY BE REQUESTED IF DEEMED NECESSARY TO SUPPORT THE CLAIM
A. General Requirements
1. Police Report and/or
2. Detailed incident report / Notarized Affidavit of Accident;
 
B. For Third Party Bodily Injury
1. Original official receipts;
2. Medical certificate;
3. Doctor’s prescription of medicine;
4. Hospital SOA (if any);
5. Proof of payment (if any);
6. Quit claim signed by third party claimant and 2 copies of Valid IDs.
 
C. For Third Party Property Damage
1. Formal claim/demand letter from third party;
2. Pictures showing the damages;
3. Repair estimate or Replacement quotation
4. Proof of repair / replacement i.e. Sales Invoice, Delivery Receipt &/or Official Receipt, (if any);

  
Note: Other documents may be requested by MICI if deemed necessary to further support the claim. In case the claim is assigned to an independent adjuster, additional documents may be requested as part of the documentation for their report.
General Requirements
1. Incident Report and /or Notarized Affidavit of Loss;

Loss Advise needed details:
o Policy Number
o Loss date
o Insured unit details (Plate number/Conduction Sticker and/or Engine Number and/or Chassis Number)
o Place of loss
o Third Party Claimant details including contact number

Documents needed for CTPL Reimbursement:
o Police Report / Notarized Affidavit / Filled-up Accident Report Form
o Original medical receipts
o Medical Certificate
o Signed Quit Claim of the Third-Party Claimant

Basic Claim Documents:
o Copy of Policy
o Copy of Driver’s License and Official Receipt
o Copy of Vehicle’s Certificate of Registration and Official Receipt
o Police Report / Notarized Affidavit / Filled-up Accident Report Form
o Estimate of Repair / Drive-Thru Inspection Report Estimate
o Pictures of the accident (if any) and/or pictures of the damages showing plate number or conduction sticker


Additional Document needed:
o Certificate of No Claim

Note: Other documents may be requested by MICI if deemed necessary to further support the claim. In case the claim is assigned to an independent adjuster, additional documents may be requested as part of the documentation for their report.
